CME Replacement Certificates

CME Replacement certificates are available for the past 5 years.


A Signed Verification of Participation Form for each course is required before Certificates will be released. If not on file, one will be emailed to you to complete prior to receiving the Certificate.


Certificates can be sent by email or US mail:

  • If you attended a course in the past 6 months and did not receive a certificate: no charge
    (If our records show that you were provided with a certificate, you will be charged     $10 for the replacement)
  • Replacement Certificate up to 3 years ago: $25 per Certificate
  • Replacement Certificate 3-5 years ago: $45 per Certificate
